Important Notes:
• A signed waiver and valid ID are required before receiving services.
• Services are only provided to clients aged 18 and above.
• Services are available for both female and male clients.
Cancellation and Rescheduling Policy:
I understand and agree to the cancellation and rescheduling policy. I acknowledge that any cancellations or rescheduling requests must be made at least 24 hours prior to the scheduled appointment time. Failure to provide sufficient notice may result in a cancellation fee or forfeiture of my deposit. If rescheduling becomes necessary, I acknowledge that I have 30 days from the original appointment date to do so. Should I fail to reschedule within this period, a new deposit will be required to secure a new appointment.
